  • RETURNING PATIENTS ?

    All returning Patient must speak with a staff person  prior to rescheduling

  • MEDICATION REFILLS

    Medication refills are addressed at your scheduled appointment. Alternatively, you can complete the medication request form on this website MEDICATION REFILL REQUEST.
    You can also leave us a voice mail clearly stating your full name, date of birth, the medication requested and your pharmacy information.
    Please note that refill requests can take up to 72 hrs to complete and those requiring prior authorization from your insurance may take even longer.

  • APPOINTMENT CANCELLATION

    The patient is responsible for keeping or canceling your appointment. All patients are encouraged to cancel the appointment 24 hours before the scheduled appointment time by calling our phone number 508 857 0765.
    For example, if your appointment is 4/1/2019 at 10 am, you must cancel by 9:59 am on 3/31/2019. Appointments canceled outside of this window are subject to late cancellation fees and charges at the provider’s discretion.

  • WHAT INSURANCES ARE ACCEPTED AT SBHS ?

    From 8am -5pm  on M, T, W,TH, F  

    From 10 am- 2 pm Every other Saturday

  • AFTER HOURS

    If you have an emergency, you are advised to Call 911 or go to the nearest ED.  However for non-emergent matters needing immediate attention, leave a voicemail and we will attempt to get back to you as soon as we are able.

  • TREATMENT ALTERNATIVES

    At SBHS we deal mainly with psychopharmacology and symptom management. We, however, do understand and recognize that medication is not the only answer or stable  Mental health and that not every patient is a candidate for medication management. We do refer patients out to alternative services as deemed necessary

  • IN CASE OF EMERGENCY

    SBHS is outpatient only in case of emergency or worsening symptoms, patients are encouraged to call 911 during emergency period or go to the nearest ED or crisis center closest to them.
